Once you step onto the field you lose your redshirt eligibility for that year. It's pretty straightforward.

The injury redshirt the NCAA will decide, but for the regular redshirt year a player is afforded, you forfeit that if you take one single snap.

Ok, I did some checking and it seems that you can only play in scrimmages and exhibitions if you are going to red shirt. A medical is different, that is up to a review board composed by the NCAA. Straight out of the Nebraska NCAA compliance office.
